Three ways to fund group health insurance

  • Fully insured: You pay an insurance company a set monthly premium, and it pays the claims. Costs stay predictable, and your workload stays light.
  • Level funded: You pay the same amount each month. Part of it covers expected claims, part buys stop-loss coverage, and part covers administration. Stop-loss is insurance that caps what your business pays when one person’s claims, or the whole group’s claims, run very high. If claims come in low, you may get part of the surplus back. [VERIFY: surplus rules vary by state and carrier]
  • Self-funded: Your business pays employees’ claims directly as they come in, usually with a third-party administrator (a company that processes claims) and stop-loss coverage. You get the most control over plan design, and your costs move with how much care your team uses.

Frequently Asked Questions

1. What is group health insurance?

It is a medical plan your business offers to employees under one policy or arrangement. Your business and your employees usually split the monthly cost.

2. What is the difference between fully insured, level funded, and self-funded plans?

With a fully insured plan, you pay the insurance company a set monthly premium, and it pays the claims. With a level funded plan, you also pay a set monthly amount, but part of it funds expected claims, and you may get a share of unused claim dollars back at year-end. With a self-funded plan, your business pays claims directly as they come in, so costs move with how much care your team uses.

3. What is stop-loss insurance?

It is a separate policy that caps what your business pays when one person’s claims, or the whole group’s claims, run very high. Level-funded and self-funded plans use it so one large claim does not become a surprise bill.

4. What does Keenly handle after we pick a plan?

We help with employee communication, COBRA administration, 1094 and 1095 reporting, and CAA compliance. We also review your HRIS and send a year-round strategic scorecard.
